More U.S. women choosing IUDs for birth control

 NEW YORK (Reuters Health) - A growing number of U.S. women may be opting for intrauterine devices (IUDs) as their birth control method, a national survey finds.


That's good news, researchers say, since IUDs and contraceptive implants are the most effective forms of reversible birth control.

In the U.S., though, they are still far from the most popular, with use lagging well behind birth control pills and condoms.

In the new study, researchers found that in 2009, 8.5 percent of U.S. women using birth control chose an IUD or implant - with the large majority going with the IUD.

That was up from just under four percent in 2007, according to findings published in the journal Fertility and Sterility.

"We saw some pretty notable growth," said lead researcher Lawrence B. Finer of the Guttmacher Institute in New York, a sexual and reproductive health organization.

It's not clear what's behind the shift, but a combination of factors are likely at play, Finer told Reuters Health.

One is that medical societies, like the American College of Obstetricians and Gynecologists (ACOG), have endorsed IUDs and implants.

ACOG says the devices should be offered as "first-line" options for most women, because of their effectiveness and safety.

Another reason, Finer said, is that advertisements have probably increased women's awareness of IUDs.

IUDs are implanted in the uterus, where they release small amounts of either copper or the hormone progestin to prevent pregnancy. The contraceptive implant, about the size of a matchstick, is inserted under the skin of the arm, where it releases controlled amounts of progestin.

The hormonal IUD, sold under the brand name Mirena, can prevent pregnancy for five years, while the copper version, sold as ParaGard, is effective for about 10 years. The contraceptive implant, sold as Implanon, works for three years.

Finer said the devices are "substantially" more effective than the Pill or condoms because they do not rely on perfect use.

"They are basically ‘set and forget' methods," Finer said.

With IUDs, it's estimated that between 0.2 percent and 0.8 percent of women will have an unplanned pregnancy within a year. The rate is just 0.05 percent with a contraceptive implant.

In contrast, the Pill and condoms must be used perfectly to be most effective. And with the way people typically use them, the unintended pregnancy rate is about nine percent per year.

With condoms alone, it's between 18 and 21 percent.

More could be done to encourage U.S. women to opt for IUDs and implants, according to Finer and his colleagues.

In some other countries, the devices are much more popular: in France and Norway, for example, about one-quarter of women on birth control use IUDs or implants. And in China, a full 41 percent do, Finer's team says in its report.

SAFETY MISCONCEPTIONS

Recent research has found that some doctors in the U.S. still harbor misconceptions about the safety of IUDs. A study by the Centers for Disease Control and Prevention found that 30 percent of health providers surveyed had doubts about the safety of IUDs for women who've never given birth.

When IUDs first came out, there were concerns that they might raise the risk of pelvic infection and jeopardize women's future fertility. So the original IUD labeling said the devices were contraindicated for women who'd never had children.

But it's now known that IUDs do not carry those risks.

"Many doctors continue to have outdated ideas about who can and cannot use an IUD," Finer said.

Cost may be another issue keeping women from using IUDs and implants.

The devices have a big upfront cost: the price of the Mirena IUD has gone up over the years and is now nearly $800. The ParaGard price tag is about $500. Then there are the doctor's charges for putting them in.

The Implanon implant costs between $400 and $800, with all charges considered.

Since the devices last for years, they are cost-effective in the long run, Finer said.

Birth control pills, for instance, cost anywhere from about $10 to $50 per month, depending on whether a woman uses generic or brand-name pills.

But if women don't have full insurance coverage of the initial cost - or have to wait to be reimbursed - they may balk at the price tag, according to Finer.

"That upfront cost can be a big hurdle," he said.

Finer said that may change, though, with healthcare reform. The new law considers birth control part of preventive healthcare, and most health plans must cover it without co-pays and deductibles.

IUDs and implants can have unwanted side effects, however. With the implant, irregular menstrual bleeding is most common; some women stop having their periods altogether.

The Mirena IUD may also cause menstrual irregularities. But it generally makes periods lighter, which is why it is also approved as a treatment for heavy bleeding. The ParaGard IUD has the opposite effect: menstrual bleeding and cramping can increase, though that may go away over time.

"There's no one best birth control method for all couples," Finer said.

But he added that now that many couples are putting off having children until their 30s, more women may want to consider long-acting birth control options.

"Childbearing has shifted to later years," Finer said. "So it makes sense to think long-term."

GENEVA (AP) — A sexually transmitted disease that infects millions of people each year is growing resistant to drugs and could soon become untreatable, the World Health Organization said Wednesday.
The U.N. health agency is urging governments and doctors to step up surveillance of antibiotic-resistant gonorrhea, a bacterial infection that can cause inflammation, infertility, pregnancy complications and, in extreme cases, lead to maternal death. Babies born to mothers with gonorrhea have a 50 percent chance of developing eye infections that can result in blindness.

"This organism has basically been developing resistance against every medication we've thrown at it," said Dr. Manjula Lusti-Narasimhan, a scientist in the agency's department of sexually transmitted diseases. This includes a group of antibiotics called cephalosporins currently considered the last line of treatment.
"In a couple of years it will have become resistant to every treatment option we have available now," she told The Associated Press in an interview ahead of WHO's public announcement on its 'global action plan' to combat the disease.

Lusti-Narasimhan said the new guidance is aimed at ending complacency about gonorrhea and encouraging researchers to speed up their hunt for a new cure.
Once considered a scourge of sailors and soldiers, gonorrhea — known colloquially as the clap — became easily treatable with the discovery of penicillin. Now, it is the second most common sexually transmitted infection after chlamydia. The global health body estimates that gonorrhea is responsible for some 106 million infections annually. It also increases the chances of infection with other diseases, such as HIV.
"It's not a European problem or an African problem, it's really a worldwide problem," said Lusti-Narasimhan.

Scientists believe overuse or incorrect use of antibiotics, coupled with the gonorrhea bacteria's astonishing ability to adapt, means the disease is now close to becoming a super bug.
Bacteria that survive antibiotic treatment due to a mutation that makes them resistant then quickly spread their genes in an accelerated process of natural selection. This is a general problem affecting all antibiotics, but gonorrhea is particularly quick to adapt because it is good at picking up snippets of DNA from other bacteria, said Lusti-Narasimhan.

"If it didn't do so much damage it would actually be a fun organism to study," she said.
Resistance to cephalosporins was first reported in Japan, but more recently has also been detected in Britain, Australia, France, Sweden and Norway. As these are all countries with well-developed health systems, it is likely that cephalosporin-resistant strains are also circulating undetected elsewhere.
"I think this is probably only the tip of the iceberg," said Prof. Catherine Ison, who oversees the national surveillance program for treatment of resistant gonorrhea in England and Wales.
The over-the-counter availability of low potency antibiotics in some Asian countries is one of the reasons why resistance is increasing, she said.

Doctors in Britain are now switching to using a combination of antibiotics over a longer period of time to combat resistant gonorrhea, said Ison, who is based at the UK's Health Protection Agency.
The Geneva-based WHO wants countries not just to tighten their rules for antibiotic use but also to improve their surveillance systems so that the full extent of the problem can be determined.
Better sex education is also needed, as proper condom use is an effective means of stopping transmission, said Lusti-Narasimhan.
"We're not going to be able to get rid of it completely," she said. "But we can limit the spread."
